Karsten, I need to look up the author of "Tedium and the software process" (ISBN 026202294X ) ("tedium" was the name of a medical database system the author built). He had another book reflecting on his career doing medical software development at Johns Hopkins University. That other book had some insight you might enjoy. Sorry that this comment isn't very useful right now.
