Anti Pattern Language

The opposite of a PatternLanguage.

Things done wrong reinforce each other. You're in trouble and you're not quite sure how you got there.

Do we need examples? (DeathMarchValues would seem to fit) Horror stories? Tips on diagnosis and cure? (reading about the PatternLanguage seems like a good place to look).
